A New York health care proxy lets you appoint a trusted agent to make medical decisions for you if you lose the capacity to communicate or decide.
The short version
Who speaks for you is a decision best made now, by you, not later by a court.
While you are capable, you make your own decisions. If you lose capacity, your agent can make treatment decisions, decide on life-sustaining measures, choose facilities and providers, and access your medical records.
Without a proxy, default rules apply and court involvement may be needed, which can delay decisions or lead to choices that don't match your wishes.
